Funeral & Burial Request Form

Please complete this form in its entirety when scheduling a burial or graveside service with the Township. Requests should be submitted at least 3 days in advance to allow adequate time for scheduling and cemetery preparation.

Opening & Closing Fees: Burials are $600 Monday through Friday and $800 on Saturday. Cremations are $200 Monday through Friday and $400 on Saturday.

There are no burials on Sundays or observed/legal holidays. Monday through Friday graveside services must be completed by 2:00 PM. If past 2:00 PM there will be a additional charge of $200.

Submission of this form does not constitute final scheduling approval. A confirmation will be sent after the request has been received and reviewed.
